Well-Led has six KLOEs. Most services prepare evidence for just one.

Well-Led is the key question most managers feel least prepared for. Here is what the six KLOEs actually assess, and what strong evidence looks like under each.

Well-Led is one of the five CQC key questions. It covers six KLOEs: Strategic direction, Workforce equity and culture, Capable and compassionate leaders, Governance and management, Partnerships and communities, and Improvement, innovation and learning. Together they assess whether leadership and governance in your service produce high-quality, person-centred care, encourage learning and innovation, and promote an open and fair culture. Why Well-Led produces the most confusion

Of the five CQC key questions, Well-Led produces the most anxiety. It also produces the most misplaced preparation. Many registered managers think of Well-Led as a documentation exercise: policy folders, meeting minutes, audit trails, risk registers. Those things matter. But they belong to one KLOE: Governance and management. The other five KLOEs assess something harder to document and easier to see: your culture.

A governance folder tells an inspector what the service intended. A Well-Led rating reflects what the service actually does, and whether staff can demonstrate that in a conversation.

The six Well-Led KLOEs explained

1. Strategic direction

Strategic direction asks whether there is a clear vision and strategy to support the current and future needs of people and promote a positive culture. Inspectors are assessing whether the service's values are understood and visible in practice. Printing them on a wall does not count. Embedding them in an induction pack does not count. At Good level, the CQC draft framework (2026) expects staff at all levels to have a well-developed understanding of equality, diversity and human rights. It also expects the vision to have been developed through co-production with people who use the service. If the manager is the only person who can articulate the service's direction, that is a gap.

2. Workforce equity and culture

Workforce equity and culture asks whether there is an inclusive and compassionate culture that values diversity, supports staff wellbeing and speaking up, and tackles workforce inequalities. This is a culture KLOE, not a HR one. Inspectors will look at whether staff feel safe to raise concerns. They will check whether speaking-up processes are accessible and embedded. They will ask what the service has done in response to staff feedback. Speaking-up processes that exist on paper but have never been used are not strong evidence. This KLOE connects directly to the work described in our post on compassionate and inclusive leadership.

3. Capable and compassionate leaders

Capable and compassionate leaders asks whether leaders at all levels have the capability and experience to lead effectively and deliver high-quality care, with accountability, integrity and empathy. The scope includes the Fit and Proper Person Requirement (FPPR), leadership development, and succession planning. At Good level, the framework expects a registered manager to be in post and to understand their responsibilities. That manager should also be actively supported by the board, trustees or directors. A registered manager who cannot clearly describe their accountability arrangements is not demonstrating capable leadership. They are demonstrating a gap in leadership support.

4. Governance and management

Governance and management asks whether there are clear roles, responsibilities and systems of accountability to support good governance and manage risks, performance and issues. This is where governance documentation is directly relevant. Regulation 17 of the Health and Social Care Act 2008 (Regulated Activities) Regulations 2014 requires providers to have an effective system to identify, assess and manage risks to people. The framework expects this system to be regularly reviewed. Staff should understand their roles within it. Data and notifications must be submitted to external partners as required.

5. Partnerships and communities

Partnerships and communities asks whether the service is working effectively and collaboratively with people who use the service and partners to support care provision and service development. At Good level, the framework expects strong external relationships and early engagement with people and communities. Services should demonstrate the ability to share learning with partner agencies. A service that does not engage with commissioners, local authority teams or community partners will struggle to evidence this KLOE.

6. Improvement, innovation and learning

Improvement, innovation and learning asks whether the service enables and embeds continuous improvement, innovation and learning, using evidence and lived experience. This is the KLOE that catches services who complete audits without acting on them. At Good level, the framework expects learning to be a continuous part of improvement. Staff should be consistently encouraged to contribute ideas. Evidence-based good practice from external sources should be embedded. If your audits consistently identify the same concerns without improvement, that is visible to an inspector. The distinction between completing audits and learning from them is precisely what this KLOE is designed to assess.

What strong Well-Led evidence looks like in practice

Well-Led evidence is different because much of it is a conversation, not a document. If staff cannot describe the service's values, do not know how to raise a concern, or have never been invited to contribute to improvement, an inspector will draw conclusions. Those conclusions affect the rating.

The practical question is not only what documents you can produce. It is whether your team can demonstrate Well-Led in a conversation. Strong preparation requires the manager to be honest about both. A service with clear governance systems but no meaningful speaking-up culture has one strong KLOE and five weaker ones. A service with an embedded learning culture and visible values but variable record-keeping has a different risk profile.

Understand which KLOEs are strong and which need work. A weakness in one is not offset by strength in another.
